The reason is that the Lady have inuried her feet with a long thorn. Marcello, as actor, extract this thorn from her feet using his mouth and his teeth (I think this scene can be good for the women foot's lovers). After this, he carry Catherine on his shoulders to his home in the island, where the story is localized.
